When this has happened to me, it's because the top thread has jumped out of
the tension assembly. Re-thread, making sure the presser foot is up while
you're threading, then tug the thread back and forth through the assembly a
little bit, to make sure it's set. If this doesn't help, you may have a
piece of thread stuck in the upper tension assembly. Floss the upper
thread paths with a thin ribbon or something like that.
